Pharmacy POS: Counseling, Compliance, and the Pickup Counter
Run a pharmacy counter that never misses a counsel, logs every controlled pickup, and keeps the front end ringing while the pharmacist verifies.
The NoPOS developer relations team writes practical guides for building on the NoPOS REST API — from first smoke test to production POS.
A pharmacy counter is two businesses sharing one register: a regulated dispensary where every handoff has a legal trail, and a front-end shop selling vitamins, snacks, and greeting cards at full retail rhythm. A generic register treats both like groceries and forces the pharmacist to keep the compliance paper in a binder beside the terminal. Build the pickup flow around the counsel and the signature, lock controlled substances to ID and log, and let the front end run fast without touching the pharmacy queue.
Never let a new script leave without the counsel offer
Every new prescription legally needs a counseling offer, and the failures happen on busy evenings when the tech hands over the bag to keep the line moving. Gate the pickup completion on a recorded counsel accept-or-decline per prescription — a single tap that logs who offered, who received, and the timestamp into the sale record. Queue the pharmacist consult as a register state, not a shout across the counter: the sale waits in "counseling" until the pharmacist clears it, so the line sees progress instead of a frozen screen. Refills the patient already knows can flow through express pickup, but keep the offer one tap away so the audit shows the question was always asked.
Lock controlled pickups to ID, signature, and log
Controlled substances turn a pickup into a chain of custody. Require photo ID capture and an electronic signature on every pickup of a flagged class before the drawer or the will-call bin releases the bag, and store both against the prescription number — not the day's receipt — so an inspector can pull the exact handoff in seconds. Enforce pickup authorization lists so a spouse or caregiver outside the profile cannot collect without a pharmacist override that names the relationship. Reconcile the will-call bin nightly: every unclaimed bag past its return window goes back to stock with a logged reversal, and the register should show the bin's aging at a glance instead of letting bags pile until someone guesses.
Keep the pharmacist's queue separate from the front-end line
Vitamins and cough drops should never wait behind a prior authorization. Run two logical queues on the same system: the pharmacy queue ordered by fill verification status, and the front-end queue ordered by arrival, with separate ready signals so each moves at its own pace. Let techs ring front-end sales on any terminal while the pharmacist verifies in the dispensary view — the verification screen shows fills awaiting check, counsel flags, and insurance rejects, never a gift-card rack. When the pharmacist is the bottleneck, every other terminal stays productive instead of the whole store inheriting the pharmacy's wait.
Make insurance rejects solvable at the counter, not the phone
A rejected claim at pickup is where pharmacy lines go to die. Surface the rejection reason on the register in plain words the tech can act on — prior auth needed, refill too soon, NDC mismatch — with the next step attached: resubmit, call the prescriber, or offer the cash price. Keep a one-tap cash-price quote so the patient hears "your copay is blocked, but the cash price is $12" instead of "come back tomorrow." Log every attempted resubmit with its response code so the rework trail is complete and the same rejection does not get billed blind twice.
Close the day with counts the inspector already trusts
Pharmacy close is part cash drawer, part drug accountability. Count every drawer against its own sales like any retail close, then add the pharmacy layer: controlled-substance dispenses reconciled against the log, register voids on prescriptions reviewed with reasons, and pseudoephedrine-style restricted sales checked against their purchase logs. Batch card totals automatically and export the day's figures to match the processor deposit, with the counsel log and pickup signatures archived by date. When the numbers, the logs, and the signatures all come from the same system, an inspection is a printout, not a project.
